How to Refer a Child
1. Initial Contact

2. Submit a Referral Form
Once suitability is confirmed, the referring social worker will need to complete and submit a referral form. This will include all necessary documentation, such as recent assessments and care plans, to ensure we have a comprehensive understanding of the child’s background and needs.
3. Assessment
After reviewing the referral, we will arrange an assessment to determine the most appropriate care plan. This process includes discussions with the child’s social worker, and if appropriate, the young person themselves. Our team will work collaboratively with you to create a seamless transition into our care.
4. Placement
If the referral is accepted, we will agree on a placement date and coordinate the child’s move into our home. Our team will be on hand to support the transition, ensuring the young person settles into their new environment comfortably.
